PACE HomeCare RN
Boulder and Adams counties, primarily Lafayette to Arvada
**$6,000 Sign-on Bonus!!!**
The Home Care Nurse provides skilled nursing care to PACE program participants within their primary residence. The nurse will work with other IDT staff in the provision of medical and nursing care, ensuring the highest quality of care in accordance with PACE policy and applicable regulations.
EDUCATION AND QUALIFICATIONS
- Must possess a current RN license in the state of Colorado
- 2+ years working as an RN
- Homecare experience, preferred
- Experience working with the frail or elderly population, preferred
- Must successfully perform position-specific competencies prior to assuming participant care as an RN
- Must successfully pass agency required background checks and drug screening
Schedule: Monday-Friday, 8 am-5 pm
Pay: $86,112 to $103,334.40 per year

PACE Certified Nursing Assistant (CNA)
Lafayette, Longmont, Dacono, Frederick, and/or Brighton
**$3,000 Sign-on Bonus!!!**
The CNA assists participants and/or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, and toileting) as assigned in a variety of settings including the TRU PACE Day Center, homes, nursing facilities, and assisted livings. Our ideal candidate enjoys working with the elderly and is flexible to care for patients in our Lafayette, CO Day Center or in homes in the surrounding area. This role’s primary focus will be on homecare for patients in Lafayette, Longmont, and nearby areas.
EDUCATION AND QUALIFICATIONS
- Must possess a current Colorado CNA license
- 1+ years of experience working as a CNA
- Experience working with the frail or elderly population
- Experience working with individuals with mental health needs
- Must successfully pass agency required background checks and drug screening
Schedule: Monday – Friday, 8 am to 4:30 pm
Pay: $22.77 to $24.84 per hour
